Healthcare Center Hemse

Hemse healthcare center is a rural healthcare center with approximately 10,000 registered patients.

Here we work in interdisciplinary teams. We have, among other things, blood pressure clinics, heart failure clinics, diabetes clinics, elderly care clinics, asthma/COPD clinics, osteoporosis clinics, lifestyle clinics, psychosocial teams, acute teams, wound clinics, and smoking cessation programs.

We are an educational healthcare center where we assist in training ST and AT doctors, nurses, district nurses, medical students, nursing students, and medical secretaries. We work with research and promote development work.

We are a creative group that is easily accessible and have developed a good structure to meet the patient group with mental health issues. We have had a project work around health guides that has led us to see a need for a combined position as a conversation therapist, who also has tasks as a health guide.

Your Tasks

With your experience and expertise, you will help our patients from an individual, group, and organizational perspective in close collaboration with other professionals. We are therefore looking for you who are a CBT therapist/social worker and who wants to work with first-line psychiatry and its preventive measures with adults. Your tasks will also involve collaboration with other actors such as schools, the Employment Service, social services, and the Social Insurance Agency, where you may participate in and initiate care planning, write certificates, and offer shorter support conversations.

You will be part of a psychosocial team with a psychologist, curator, rehab coordinator, occupational therapist, and physiotherapist in collaboration with doctors and other staff.

Who Are You?

You who are applying are a licensed social worker/CBT therapist and have experience in primary care or inpatient care or other equivalent independent treatment work. It is meritorious if you have experience working as a health guide or care coordinator.

You have an interest in and experience working with evidence-based methods and a willingness and openness to further develop our treatment work within the organization. You thrive in collaboration and working in a group.
